    HOUSING PROGRAMS

    Continuum of Care (CoC) & HOPWA Housing provides permanent supportive housing and supportive services to eligible persons.

  • ✔We provide rental assistance and case management services to individuals experiencing homelessness, who have disabilities, housing instability, and who are living with HIV.

    Referrals are received through service providers, self-referrals, and Coordinated Entry Access Point.
